A customer request form is used in banks to let customers use it for various requests. Most common requests like requesting a cheque book, knowing the status of debit card request, standing instructions, changes in term deposits, change of personal information in bank account and the like can be made using this form.
Almost every bank has this form – only the design, pattern and some info may vary.

Information you that I’m using canara bank past 12 years onwards, my request is I visited branch many time to apple personal loan not responding properly.(CANARA BANK – BENGALURU CHAMARAJPET – 560 018 )
I’m not yes bank account holder but there prodding loan form me, then why we want to continue canara bank.,
I’m not satisfied with canara bank service. Please take serious action of my complaint.
